Appointment Rules
New Patients
Please allow 10 minutes for new patient registration and consent forms to be filled out completely and correctly.
Please bring along photo ID to confirm your identity and age.
You will require a consultation prior to any procedures being performed and no guarantees are made you will be accepted as a new patient or your chosen procedure will be performed.
If you would like to use a different payment method please discuss this prior to booking online and paying the online booking fee.
All Patients
Patients under the age of 16 MUST be accompanied by a parent or legal guardian, additional persons attending the appointment are not being consulted unless both patients have booked concurrent appointments. Children will not be permitted entry into the clinic room for procedure appontments due to health and safety of the patient, practitioner and the child.
